This page must be completed and signed by a parent/guardian for student participation in the Hamilton-Wentworth and Halton Science & Engineering Fair, April 5-8, 2000 at Mohawk College. In the case of group projects, a separate release form must be signed by a parent/guardian for each student in the group. Please FAX completed form(s) as soon as possible to the HWHSEF at 905-577-9099. Please note that Intermediate and Senior Students (grade 9-OAC) have additional safety forms that need to be completed and faxed to the HWHSEF. Please fax all forms together. |
As a parent/guardian, I certify, to the best of my knowledge, that the information on this HWHSEF Fair 2000 application for registration is correct and the project is the work of the student. I also realize that the material used in the project is the responsibility of the student and that neither the school, nor the teacher, nor the HWHSEF is responsible for damage or theft.
I agree that the exhibit will be left on display until the end of the Fair. If it is not removed, the HWHSEF is free to dispose of the project as seen fit.
I hereby authorize and agree to the using this student’s name and photograph for the HWHSEF web page and future HWHSEF publications.
I have read and agree to all stipulations expressed in the entry form. I agree that the decisions of the judges and the HWHSEF Committee will be final.
